# Employee performance review template built around evidence

## Use this for a routine manager review, not a hidden employment case

This template helps a manager organize one employee’s routine performance evidence. It does not choose a rating, decide pay or promotion, prepare discipline or termination, investigate a complaint, or repair a review process whose standards were unclear or inconsistently applied.

Stop and use qualified HR or employment counsel when the record involves protected activity, discrimination or retaliation concerns, accommodation, medical or family leave, a complaint, investigation, union rights, disputed facts, discipline, a performance-improvement plan, or termination.

The [U.S. Equal Employment Opportunity Commission](https://www.eeoc.gov/employers/small-business/5-im-conducting-performance-evaluations) tells small employers to communicate standards, apply them consistently, explain whether the employee met them, and include relevant factual details. A phrase bank cannot supply those facts after the event.

Continue only when the role and review period are defined, the expectations were actually communicated, examples can be checked against dated evidence, and material feedback was discussed before the deadline.

## Build the evidence before you write the review

The interactive worksheet on the canonical page sends no information to Fidelic. Use a role label rather than an employee name. Keep medical facts, accommodations, leave details, complaints, protected activity, investigation records, demographic data, pay records, and other sensitive personnel information in approved systems.

### 1. Role label

Use the role or an internal ID, not the employee’s name. The label should be specific enough to connect the work to the correct expectations without creating another personnel record.

### 2. Review period

Name the exact start and end dates. A material example belongs in the review only if it falls within the period or is clearly identified as prior context.

### 3. Expectations in force

Copy the approved goals, responsibilities, and standards that actually applied during the period. Include the source or record location. Do not quietly grade against an expectation that changed later or was not communicated.

### 4. Documented outcomes and observable work

List dated work, observable behavior, results, and evidence locations. Separate the employee’s contribution from the team outcome. A useful entry names the situation, the work, the result, and where another reviewer can check it.

### 5. Feedback already given

Record prior praise, correction, coaching, the date discussed, and the employee’s response or follow-through. A routine review should summarize the period, not reveal a hidden case assembled at the deadline.

### 6. Relevant work context

Record changed priorities, resources, dependencies, added responsibilities, and support. Keep medical, complaint, protected-activity, and other sensitive facts out of this general worksheet.

### 7. Proposed next work, support, and check-ins

Name the next outcome, support or owner, evidence of progress, and check-in date. Keep pay, promotion, discipline, and termination decisions outside the worksheet.

The exported packet is an evidence record, not a completed evaluation. The manager remains responsible for accuracy, consistency, the final narrative, the rating, and every decision affecting the employee.

## Separate the record, the reading, and the next work

### Record: what was expected and what happened

Use the standards in force during the period, dated work, observable behavior, results, and links or record IDs. Distinguish the person’s contribution from the team outcome.

### Reading: what the evidence means

The manager explains whether the work met the stated expectation, checks consistency with prior feedback, names relevant work context, and resolves contradictions before submission.

### Next work: what changes after the conversation

Set the next outcome, support, accountable owner, evidence of progress, and check-in date. A review that changes nothing is only a historical note.

The [U.S. Office of Personnel Management’s 2025 Measuring Employee Performance Quick Guide](https://www.opm.gov/policy-data-oversight/performance-management/performance-management-toolkit/best-practices/measuring-employee-performance-quick-guide.pdf) recommends specific observations and a situation-behavior-impact structure. It is federal-workforce guidance, not a private-employer rule, but the writing principle travels well: observable facts are more useful than labels.

## Replace the adjective with the event and its effect

“Communication needs improvement” is too vague. The reader cannot see the situation, expected behavior, result, or whether this was already discussed.

An evidence record might say: “The May 12 and June 3 launch updates omitted the revised delivery date.” Add the source, downstream effect, expectation in force, and date of earlier feedback. Remove the example if the record does not support it.

The next work might say: “For the next two launches, update the date and owner before the Friday customer brief.” Add who supports the work, what proves completion, and when the manager checks progress.

The example shows a writing method, not a conclusion about a real employee. The manager must determine what happened, what it means, and whether the example belongs in the review.

## Polished prose is not the same as a credible review

In a 2017 analysis, Gallup reported that:

- **14%** of employees strongly agree their performance reviews inspire them to improve.
- **29%** strongly agree their performance reviews are fair.
- **26%** strongly agree their performance reviews are accurate.

These are attributed 2017 Gallup findings. They describe a trust problem; they do not prove what caused it in any one company. An evidence record cannot create fairness. It can expose unsupported claims and contradictions before submission and give the employee a more inspectable explanation of the manager’s conclusions.

## The deadline arrives before the evidence does

In authenticated Reddit research across 126 live threads, people repeatedly described reconstructing ten or twelve months of work just before the deadline. One employee described “scrolling through 10+ months of emails” to remember what a March project actually achieved. The most hostile reactions were not to AI-assisted editing by itself. They were to judgment without evidence or proofreading. One employee reported an “Inconsistent” rating beside prose that called the person a “pillar of consistency.” The defect was visible before any argument about AI: the document contradicted itself. Another employee described an unedited ChatGPT review with generic language and obvious stylistic residue. The manager had saved writing time by spending the employee’s trust. The constructive advice across the corpus was steadier: keep a work record throughout the period, attach claims to evidence, distinguish self-review from manager judgment, and make the conversation about development rather than surprise. Reddit cannot tell us how common these failures are. It tells us how people describe them, which mistakes provoke distrust, and which peer practices have survived a bad experience.

Before submission, check four things:

1. **Source.** Can every material example be checked?
2. **Consistency.** Do the examples, prose, and rating tell the same story?
3. **Prior notice.** Was material feedback discussed during the period?
4. **Next work.** Does the employee know what to do and what support exists?

## Choose by where the evidence should live

The worksheet is free. The purchasing decision begins when the company asks who keeps the evidence current, who can see it, who checks consistency, and who owns the next review cycle.

### Prepare the review yourself

This fits when the team is small, expectations are already documented, the manager has direct knowledge of the work, and one or two reviews are due. Use the worksheet, audit the sources, write the manager’s reading, and keep the approved record in the company’s people system.

### Use performance-review software

This fits when the company needs repeatable cycles, reminders, permissions, goal and feedback history, cross-manager rating review, and a shared system rather than one finished document. Compare the evidence path and data terms, not just the writing feature.

### Hire an accountable owner

Use qualified HR or employment counsel for policy, employee relations, protected activity, sensitive facts, disputes, discipline, or termination. Use LILA when the routine record and manager follow-through are defined but nobody keeps them current between review deadlines.

## Use Claude as an evidence editor, not the manager

Claude can organize approved source material, identify missing support, test a draft for contradictions, and format manager-written conclusions. It should not reconstruct a year from adjectives or make the employment decision.

1. **Define the job before opening Claude.** Gather the role description, expectations in force during the period, goals, dated work records, prior feedback, relevant business results, and the company’s review form.
2. **Prepare a sanitized evidence packet.** Use a role label rather than the employee’s name. Remove medical, accommodation, leave, complaint, investigation, protected-activity, demographic, pay, identity, and other sensitive facts unless an approved enterprise process and qualified reviewer require them. 3. **Ask Claude to structure, question, and cite.** Require separate fields for expectation, event, observable work, result, evidence location, prior feedback, context, and proposed next work. Require “not supported by the supplied record” whenever evidence is missing. Ask for questions, not invented detail.
4. **Run a contradiction pass.** Ask Claude to compare every summary statement with each example and any proposed rating, then list mismatches without resolving them. The manager must decide what the evidence means and correct the source record or prose.
5. **Draft from manager conclusions.** Once the evidence and reading are approved, ask Claude to format the manager’s conclusions in the company’s form. Anthropic documents [file uploads](https://support.claude.com/en/articles/8241126-upload-files-to-claude) and file creation; those capabilities do not make the review accurate or lawful.
6. **Read it aloud and compare it with prior conversations.** Remove generic praise, personality labels, unexplained intensifiers, and claims the employee has not heard. Check that the next work names support, evidence, and a date.
7. **Move only the approved result.** Put the final review and source references in the approved people system. Do not leave the only evidence copy in an AI chat or a manager’s private notes.

A useful instruction is: “Use only the supplied record. For each statement, show the expectation, dated evidence, result, prior feedback, and missing facts. Mark unsupported or contradictory claims. Do not assign a rating or advise on pay, promotion, discipline, or termination.”

Its current help page says it does not suggest final ratings or promotion readiness. [Lattice Writing Assist](https://help.lattice.com/hc/en-us/articles/26263030050839-Lattice-AI-Reviews-Writing-Assistant) refines review text; Lattice’s help page currently says text entered into the feature is shared with OpenAI after the administrator accepts the required addendum. These are different data paths and operating models. Verify the product that will hold the employee record.

A human HR consultant or employment lawyer becomes the better route when the company needs process design, policy interpretation, employee-relations judgment, an investigation, or advice about protected rights and employment action. [Upwork currently reports](https://www.upwork.com/hire/hr-consultants/cost/) worldwide historical HR consultant contract ranges of $29–$75, $75–$100, and $100–$150+ per hour across three experience bands. These are marketplace observations, not a quote; credentials, location, scope, urgency, and risk change the price.

## What this template cannot make true

- A clean template cannot repair standards that were unclear, changed without notice, or applied inconsistently.

- The worksheet should not hold names, medical facts, accommodations, leave details, complaints, protected activity, investigation records, identity data, or other sensitive personnel information.
- Claude and review software can summarize incomplete or biased source material with unwarranted confidence. A polished draft still needs line-by-line manager review.
